Do you have blank forms?
Answer: We have basic government issued forms, such as subpoenas, summonses, in forma pauperis forms, etc. Please refer to the Forms & Publications page for downloadable forms on this website. Forms are also available in each Intake Office or by mail. There is no fee for forms, but please provide a self-addressed, stamped envelope when requesting forms to be sent to you by mail.
Do you have sample forms of pleadings?
Answer: No. The Clerk’s Office cannot provide legal advice or assist a litigant in preparing a pleading.
